The following is a list of basic requirements for most phlebotomy training programs. It’s important to understand that different phlebotomy training programs may have additional entry requirements.

  • Must be 18 years of age or older.
  • Must have completed high school or have a GED.
  • Must be able to pass a criminal background check and drug test. This may vary from program to program.
  • Join an accredited phlebotomy training program.

Phlebotomy Training in West Virginia – How To Obtain Phlebotomy Certification

Enrolling in classes is your first step. Make sure you join a NSU phlebotomy course ASAP to complete these tasks:

  • 20 hours of basic lessons
  • 20 hours of advanced lessons
  • 40 hours of hands-on training in a clinic
  • 50 successful venopinctures on live patients
  • 10 successful epidermal punctures on live patients
  • Retrieve your certificate of completion from your training program.
  • Take the national phlebotomy exam.
  • After the exam is finished, pay your app. fee online. Sign and send the atteststation page to the West Virginia DHS-LFS center. 
  • Send over the necessary high school transcripts to DHS-LFS. 
  • Make a copy of each completion certificate you’ve received.

Submit by mail all of these documents listed below to DHS-LFS:

  • A complete CPT application that you’ve filled out.
  • Copies of your phlebotomy certification.
  • A check written for $54.00 or you can use a money order instead.
  • 2 picture ID’s (passport size only).
